*All applicants must complete an online application. If you have Healthcare
Certificates or Military experience, please provide a copy of certificate
or DD 214 for Military Experience in your application packet.
Items turned in separately will not be accepted, unless indicated below.
Failure to submit required items will place your application as incomplete
and will not be considered.
Step 2: Application Checklist and Acknowledgement Form
This form must be included in your application packet.
The Resume/CV should include current contact information, educational experience
related to healthcare, work experience related to healthcare (listing
dates mm/yyyy of employment for each), and any volunteer or shadowing
experience. Also include any community service hours or special recognition
you have earned.
Step 4: High school diploma or GED verification
CSON requires an Official sealed high school transcript, verified GED or
successful completion of an approved home school program.
Step 5: All Official unopened college/university transcript
Official sealed transcripts
from all colleges and universities attended to date must be submitted with the Application for Admission. They must
be submitted with all application documentation by the application deadline.
If a student applies before all prerequisites are completed, all final
official transcripts that reflect any outstanding course work are due
by the date stated in the admission packet. Covenant School of Nursing
and Allied Health will give consideration of credit from other institutions
accredited by an agency recognized by the United States Department of
Education or the Council for Higher Education Accreditation for
prerequisite courses only.
Each candidate must submit 3 reference forms along with the application
packet. Evidence of the applicant's character, personality, and other
qualities that are important in predicting probable success in the program
will be substantiated by recommendation forms completed by individuals
selected by the applicant. References from family members will not be
accepted. The form must be sealed in an envelope by the evaluator with
a signature across the envelope.
CSON utilizes the TEAS V pre-entrance examination as part of the admission process.
The exam is offered on several dates during each application period. Candidates must score the required scores listed on the Entrance Exam tab
(click here for more info) and on all four subsections
to qualify to apply. The scores are valid for one year and scores submitted with the application
packet must be valid on the last day of the application period.
IMPORTANT NOTICE TO APPLICANTS: Please be aware that all of our students are required to have completed
the bacterial meningitis (if 22 or under) and HEP B series before the
start of school. If you have not yet begun to receive this series, or
if you will not have it completed before classes begin, you will not be
eligible for admission.
Required vaccinations include:
- Tetanus booster (td or Tdap vaccination) within the past 10 years;
- Two measles, mumps, and rubella (MMR) immunizations, or titers proving immunity;
- Hepatitis B (HBV) immunization series completed, in progress, or a Hepatitis
B Vaccination Declination Form signed by the student; Lab results showing
immunity to varicella (chickenpox) or vaccination series (two doses required);
- One Mantoux Tuberculin Skin Test (TST) within the past year or immediately
prior to program enrollment.
- Annual flu shot required.
Step 9: Application Fee
An application fee of $50 must be turned in along with your application.
The application fee must be paid by check, credit/debit card, or money
order. Cash is not accepted for this fee.
IMPORTANT NOTICE TO ALL APPLICANTS:
Proof of Permanent Residency
Unfortunately Covenant School of Nursing and Allied Health is not equipped
to accept foreign students. Therefore all applicants must be U.S. Citizens
or have established Permanent Residency. If you were born outside the
United States you will be asked to provide proof of citizenship or permanent
residency. Example of identification accepted would be permanent residency
card, certificate of naturalization, or a United States Passport.
Letter of Standing
If you have begun a program of nursing or allied health in the past and
not successfully completed it, you will need to request a letter of standing
from that program. This letter simply states that you are eligible to
apply to their program again in the future. The letter must be mailed
directly to the school.

Registered Nursing Program
CSON requires a letter of eligibility from the Texas Board of Nursing if
a petition for Declaratory Order is required related to any criminal charges
or convictions including misdemeanors (with the exception of simple moving
violations). Contact the Board of Nursing for details about submitting
a declaratory order to obtain your Letter of Eligibility
CSON will not require a background check until the applicant has been accepted
Submit your application packet
You can deliver your application packet in person. Please note that it
is not possible for our office to review your application packet at the
time it is submitted.
Please make sure to submit your documents in a manila envelope (click

If mailing your application packet please be aware it must be received
by the deadline (postmark by the deadline will not qualify your application).
To ensure we receive it by mail we recommend you send it certified.

After the application deadline, Covenant School of Nursing will consider
all qualified applicants whose application files are complete and up to
date. Selected applicants are then sent an email regarding their status
as accepted, alternate, disqualified or declined admission. Notices are
sent 4 weeks after the application deadline. We cannot give any admission
status updates over the phone or to family members. Accepted and alternate
candidates will be required to complete all of the requirements according
to the deadlines listed in their admission packet.

Are volunteer hours considered? The combined hours/work experience will be considered if it equals 1 year
or more included with your healthcare experience.
FAQ Step 4. Official High School Transcript
My school will not release my official high school transcript to me? You may have the school district mail your official high school transcript
directly to our office, Covenant School of Nursing, Attn: Admissions Office,
2002 W. Loop 289, Suite 120, Lubbock, Texas 79407.
How will you know that my high school transcript is being mailed to Covenant
School of Nursing? Please include a note that your official high school transcript will be
mailed to CSON. Please note if your high school transcript is received
after the admissions deadline it
will not be accepted. Plan ahead and request your documents in a timely manner.
I attended more than one high school? You will only need to submit the official high school transcript from
the school that you graduated from.
What documentation do I need if I complete my GED? Please note that if you completed a GED outside the state of Texas you
will need to research as to how to obtain documentation per state requirements.

FAQ Step 5. Official College or University Transcripts
I attended more than one college or university do I need to submit all
transcripts? You are required to submit all and most recent official transcripts (regionally
accredited) college or university.
I attended an out of state college or university? You are still required to submit your official college and university
transcripts even if it is an out-of-state college or university.
What if my current official transcripts list all the other colleges and
universities that I have attended? Not all official transcripts indicate the courses completed at another
institution. We must verify courses or programs of enrollment from other
colleges and universities.
What if I am having problems obtaining my college or university transcripts? Only official transcripts will be accepted, we highly recommend you start
early in researching the process of obtaining your official transcript/s.
Will an unofficial transcript be accepted? We will NOT accept unofficial transcripts for the application process.

FAQ Step 6. Reference Form
Can a relative complete the reference form? No, the reference form cannot be completed by a relative.
How do I get my reference form to the evaluator? Make sure to communicate with the person you have selected to complete
your reference form. Provide them a deadline and method to get the completed
form sealed with their signature across the envelope.
Who should I select as a reference? Here are some suggestions, a co-worker, supervisor or professor. Please
make sure to read over the information being asked of the evaluator.
FAQ Step 7. TEAS V (entrance exam)
What is the TEAS V? The Test of Essential Academic Skill (TEAS) is the assessment used as
the entrance exam for Covenant School of Nursing (CSON). The TEAS assessment
is designed to measure essential skills in the academic content area domains
of Reading, Mathematics, Science, English and Language Usage. These entry
level skills were deemed important for nursing program applicants by a
panel of nursing program curriculum experts.
Where can I take the TEAS V? The Test of Essential Academic Skill (TEAS) can be taken at Covenant School
of Nursing. It is offered during the application cycle.
How many times can I take the TEAS V? You are allowed to test at our location twice per application cycle.
Can I submit more than one score? You may only submitted ONE score sheet for the TEAS V that includes all
four sections of the TEAS V.
How long are my scores valid? Scores are valid for a year, if they are expired at the time of the application
cycle applicants must submit new scores.
When is the TEAS V offered at Covenant School of Nursing? The TEAS V dates will be posted on our website and offered during the

Where can I get a copy of my immunization records? Here are some recommendations as to where to look for your immunization/vaccine records.

How do I pay for my application fee? You can pay by personal check, money order or debit/credit.
Where can I get a money order? A money order can be purchased at the place that you bank, convenience
store or at the United States Postal Service.

How do I request a letter of standing? From the Allied Health Program that you failed or withdrew from. They

Do I need to submit an original document? A (clear readable) copy can be submitted.
