The patient portal is a secure, convenient way to manage your personal health care online. This FREE self-health management tool can document important health care information, and ultimately give you the ability to make more informed decisions about your health.

View your health information electronically via the web or FollowMyHealth Mobile - the free smartphone/tablet app available for iPhone, iPad and android users. When accessing via the app, use your same login to access your account and take advantage of all the same great features.

Your health information is posted to a highly secure data registry.


  • Review and update your health history and personal health record online.
  • Download, fax, and email select health information – immunization history and more.
  • Review your personal medical record including office visit summaries, conditions, medications and more.
  • View test and lab results.
  • Manage personal health journal.

The Online Patient Portal should never be used to report life-threatening or urgent medical condition.


If I am an existing follow my health user, do I need to sign up again?

NO. In order to start the registration process and get connected to your medical information, follow these steps when you click the registration link within your e-mail invitation:

  1. Click Sign Up and Connect. If you already have a portal account and want to connect with an additional provider, click Sign in and add this connection.
  2. Connect your account. Follow the on-screen prompts to complete your account connection. These screens include accepting our Terms of Service, entering your Invite Code [the last four digits of your year of birth], accepting the release of information, and HIPAA agreement.

How do I sign up?

You will be sent an e-mail invitation upon departure from the ED or discharge from the hospital. Please follow the enclosed instructions to complete your registration and/or connection.

Forgot usename/password

If you have previously established a FollowMyHealth user name and password or forgot which alternative verification account you used, please call (877) 888-8773 for further assistance.

Can a parent/guardian set-up and manage an account for a child/dependent?

Yes, for patients under the age of 12, parents are able to manage an account on behalf of a child/dependent. For patients between the ages of 12-17, due to state and federal regulations, the patient will be required to register in the hospital. At that time, the patient will be given the option to grant proxy access to their parent/guardian.
